Is it possible that my kitten is malnourished?

Probably; the people you got her from were probably NOT taking very good care of her. After you had her a month she probably got back onto a healthy diet; and got normal vet vaccinations to protect her from sickness. Yes; I'm guessing she was malnourished when you first got her.
